IME Requests/Services
Request an independent medical exam (IME) or file review by using our online order forms. Log in with your username and password, and your information will automatically be added to your request form. Don't have a username? Register for an account.
You may also place your request by calling us at 952.253.6600 or toll-free at 800.804.4008, or fax us 952.253.3900. We will respond shortly via fax or email with both a confirmation and fee information. Then simply send us the pertinent medical records, along with your cover letter or completed "Request for IME" or "Request for File Review" form. After the IME or file review is completed, Woodlake Medical reviews the physician's report for quality assurance and provides you with the resulting documentation.
